add hyperlink with the URL hardcoded in the code

There is a column whose values should be suffixed with a common URL and add hyperlink to it. Can some one help in getting this through macro?

Sample sheet attached;

1st sheet has the original raw table.

2nd sheet has the table which I need as a output.
